Longevity medicine is the category where the distance between what is claimed and what has been demonstrated is widest, and where the claims are hardest to test. That is not an accusation against the clinics listed here. It is a structural feature of the field: an intervention that claims to extend healthy lifespan is making a claim about decades, and no clinic can have run that experiment.
What longevity programmes actually consist of is usually familiar — laboratory panels, sometimes biological-age estimates, lifestyle and sleep work, supplements, hormone optimisation, and peptides marketed for cellular or mitochondrial function. Some of those components have good evidence for specific outcomes. The longevity framing is a claim layered on top of them, and it is generally the least evidenced part of the offering.
Biological-age testing deserves particular scepticism, not because the underlying science is worthless but because the commercial versions vary enormously in method and are frequently presented with more confidence than they warrant. A number that moves after an intervention may reflect a real change, measurement noise, or the properties of the algorithm. Ask what the test measures, how reproducible it is, and what a change in the number has been shown to correspond to.
Peptides marketed for longevity — epitalon, MOTS-c, and similar compounds — are among the least well characterised in human use of anything in this directory. Several have no identified human exposure data at all. That is not a claim that they are dangerous; it is a statement that the information needed to judge is not available, which is itself the relevant fact when someone proposes you take one indefinitely.

Where a programme's central claim is about outcomes decades away, the questions worth asking are about the parts that can be checked now: who is prescribing, what is being measured, what is being supplied, and what happens if something goes wrong.
Longevity programmes tend to be expensive, and the cost is not always visible at the outset. The consultation may be modest while testing, supplements, peptides and follow-up carry the real expense, often on an annual cycle. Ask for a full year's projected cost including everything before committing to the first appointment. A programme confident in its value will give you that number; one that will not is telling you something about how it expects to make money.
The evidence base is not uniformly weak, and treating it as uniformly weak is its own error. Sleep, resistance training, cardiovascular fitness and specific managed risk factors have substantial support for outcomes that matter. Those components are also the least profitable, which is why they are frequently mentioned briefly and then surrounded by the parts that are not well supported. A programme weighted toward the evidenced components and light on proprietary products is a better sign than the reverse.
Peptides marketed for longevity deserve a separate judgement from the rest of the programme. Several are sold on the strength of mechanistic reasoning — a pathway implicated in ageing, a molecule that acts on it — without human outcome data of any kind. Mechanistic plausibility is a reason to run a trial, not a reason to take something indefinitely. Ask what human evidence exists for the specific compound and route, and treat the absence of an answer as the answer.
A closing note on how to read this category. The strongest signal of a serious longevity practice is not the sophistication of its testing but its willingness to say that something is unproven. Uncertainty is the honest state of most of this field, and a clinic that can distinguish what it knows from what it hopes is applying the standard that matters. Confident answers across the whole range of what these programmes offer should lower your trust, not raise it.
Regulatory status
Several compounds marketed in longevity programmes are neither approved nor eligible for compounding, and some appear on FDA lists identifying substances that may present significant safety risks. A substance leaving one of those lists does not make it legal to compound; nomination categories are administrative positions, not approvals. Check each compound individually and by name.
What to establish first
Five things worth settling before a consultation. None of them require clinical knowledge to ask.
- 1What outcome the programme claims to change, and what evidence is offered for that specific outcome.
- 2If biological age is measured, what the test measures and how reproducible it is between runs.
- 3For each peptide offered, whether human exposure data exists for that compound and route.
- 4What the programme costs annually, including testing, and what is excluded.
- 5Whether a named clinician reviews the plan, and how side effects would be handled.

It depends on the method, and commercial tests vary widely. Ask what is measured, how reproducible the result is between runs, and what a change has been shown to correspond to. A number that moves is not by itself evidence that anything clinically relevant has changed.
