Corrections policy
Anyone may propose a correction with a public supporting source; every proposal is moderated before public data changes.
Submitting a correction
Identify one field, provide a proposed value, and link to supporting public evidence. Do not submit patient information, diagnoses, symptoms, medications, dosages, lab results, or emergency information.
Review
Editors evaluate the source, record a decision, and may request more evidence. Approval of a correction request does not itself publish a change; application and verification are separate steps.
